Designing a Connected Patient Journey Across Digital and Physical Care
Patients experience one journey even when the organization delivers it through many channels, teams, and systems.
A patient may begin with a search, move to a website, schedule through a portal, receive instructions by text, arrive at a physical location, speak with several teams, and continue care through digital follow-up. Internally, those moments may belong to different departments. To the patient, they are one experience.
Map the journey around the patient's goal
Begin with what the person is trying to accomplish: understand a concern, find appropriate care, schedule, prepare, receive service, manage treatment, or get support. Organizational boundaries should not define the journey.
Make handoffs visible
Transitions between channels and teams create the greatest risk of confusion. Ownership, status, next steps, and required information should travel with the patient rather than requiring the patient to reconstruct the process.
Use communication as navigation
Messages should explain what is happening, what the patient needs to do, and where help is available. Coordinated communication reduces uncertainty and prevents avoidable service demand.
Measure the whole experience
Access, abandonment, time to service, completion, no-shows, repeat contacts, understanding, satisfaction, and clinical or service outcomes reveal where the journey supports progress and where it creates friction.
